About the role

cohealth's Family Recovery Program is newly created s been established to support culturally diverse families. This support is for employment, education, mental-health recovery alth promotion after a positive COVID- diagnosis or a public health order to self-isolate or quarantine. The Case Manager will support improved access, connection to vigation of, mainstream her culturally specific support services related to education aining, employment, health ntal health VID- health promotion.

Your daily duties will vary but will focus on:

Undertake a culturally sensitive comprehensive needs assessment with referred families to identify support needs als
Ensure a trauma informed ltural
safe approach to undertake assessment a recovery plan
Act as the main point of contact for the family ovide supports to coordinate their care
Actively engage with the family:
using persistence, compassion, flexibility, underst of trauma, to build trust intain a relationship

About you:

Demonstrated experience working in community, settlement or public health settings
Experience in working with consumers from CALD or disadvantaged communities
Demonstrated success with delivering service which is trauma informed lturally appropriate
High level communication terpersonal skills ility to work effectively with managers, staff akeholders in a state-wide context
Experience in delivering health/community services using a Human Rights approach.
