About the role

The role of the Service Navigator exists to improve accessibility to the health system for individuals with severe mental illness sociated psychosocial disordershe Service Navigator will also be one of the main points of entry, intake sessment for psychosocial supports across the Barwon eater South Coast regionsp>

Working closely Local Health Networks IS Local Area Coordinators the Service Navigator will work to facilitate consumer access hance coordinated referral processes through the creation stribution of detailed resources relating to community-based support services as well as psychosocial, clinical imary healthcarep>

The successful applicant will work alongside Wellways Barwon eater South Coast Psychosocial support programs take teamss Service Navigator you will engage with vocate for individuals, connect them with the appropriate support networks power them on their recovery journeyp>

Building awing upon Wellways' Community Recovery Model, the Service Navigator will establish hance coordinated referral processes as well as community support services focused on psychosocial support needs such as housing, employment, education, family support nancial matters

What do you need-

Relevant qualification or commensurate experience in working in a support role for people with mental illness, a Diploma IV qualification or above
Current valid Driver's License
Evidence of the right to work in Australia
Evidence of, or a willingness to undergo a National Police Records Check, Working
with Children Check-Employment, NDIS Worker Orientation Module certificate IS Worker Screening Check clearance
