About the role

This position sits in the AOD melessness Cluster, which aims to improve the health, wellbeing, housing cial outcomes of people who use alcohol her drugs those experiencing homelessness in the Inner North West of Melbournehe Homelessness to a Home program will support consumers currently in emergency accommodation to exit into affordable housingp>

Your daily duties will vary but your main focus will include:

Assist clients to access, transition tablish suitable longer-term housing
Comprehensive assessment, care plans, referral, co-ordinating case
conferences, maintaining client records rticipating in case discussions
Build relationships rk collaboratively with specialist services
Advocate on behalf of clients with other agencies to ensure access to services

About you:

A relevant tertiary qualification in Social Work or equivalent qualification perience in the housing, mental health, disability or welfare sectors
Relevant professional lived experience as well as a sound underst of the key issues affecting people experiencing homelessness
Experience in care planning, comprehensive assessment, intake, advocacy ferral with clients who have a history of homelessness ltiple mplex needs
Demonstrated experience in undertaking, supporting ordinating assertive outreach oup activities
